Abstract

POLITEKNIK KESEHATAN KEMENKES BANDUNG PROGRAM STUDI KEBIDANAN BOGOR FINAL REPORT PROJECT, JULY 2021 Post partum midwifery care for Mrs. S 28 years P2A0 with retained placental bleeding at Sekarwangi Hospital Revi Andriani Koswara NIM : P17324218008 VI chapter, 44 pages, 7 appendices, 0 pictures, 2 tables ABSTRACT Results of the inter census survey (SUPAS) the maternal mortality rate (MMR)in 2015 in Indonesia was 305/100,000 KH. The cause of the highest maternal mortality rate is bleeding as much as 30.3%. Post partum bleeding is one of them caused by the rest of the placenta as much as 23-24%. Therefore this should receive special attention. The purpose of writing this final report is to study, understand and apply midwifery care in cases of postpartum hemorrhage due to retained placenta. The method used in writing this final project is a case report with documentation in the form of subjective, objective, analytical, and management (SOAP). The time and place of taking the case will be on Wednesday, April 7, 2021 at the Sekarwangi Hospital, Sukabumi. Subjective data assessment, Mrs. S aged 28 years P2A0 came with complaints of dizziness and profuse bleeding from the birth canal, Mrs. S was referred by the puskesmas with a diagnosis of primary post partum hemorrhage, impression of retained placenta. Assessment of objective data Mrs. S, general condition is weak, blood pressure 90/60 mmHg, pulse 90x/minute, respiration 23x/minute, uterine fundal height 1 finger below the center, inadequate contractions, there is bleeding ± 500 cc on the underpad, ultrasound examination stated that there was a small portion of the placenta (membrane) and the maternal Hb examination was 11.7 g/dL. The analysis obtained is Mrs. S aged 28 years P2A0 with bleeding due to the rest of the placenta. The management carried out was administration of uterotonic fluids in 500 cc RL, administration of antibiotics, blood-added tablets, and care during the puerperium. The care that was carried out was in accordance with the procedures and theories, Ny.S's bleeding had stopped and she went home in good health. Suggestions for the hospital to maintain services and advice for Mrs. S to better understand the danger signs during the puerperium,nutrition, rest, hygiene and family planning after the postpartum period is over.
