Abstract
Exposure to cigarette smoke remains a public health problem affecting both active
and passive smokers. Passive smokers are at risk of exposure to various harmful
substances contained in cigarette smoke, including nicotine, which is metabolized
into cotinine in the body. Cotinine is a biomarker that can be used to identify
exposure to cigarette smoke. This study aimed to describe urinary cotinine as a
biomarker of cigarette smoke exposure among passive smokers using a qualitative
approach. This study employed a descriptive method with a cross-sectional design.
The sample consisted of 25 final-year students who met the criteria as passive
smokers. Data were collected through a questionnaire regarding cigarette smoke
exposure and qualitative urinary cotinine examination using a strip test method.
The data were analyzed descriptively and presented as frequencies and
percentages. The results showed that 9 respondents (36%) had positive urinary
cotinine results, while 16 respondents (64%) had negative results. Positive results
indicated the presence of nicotine exposure from environmental cigarette smoke
that entered the body and was metabolized into cotinine. These findings suggest
that cigarette smoke exposure still occurs among some passive smokers despite
having no smoking habit. In conclusion, qualitative urinary cotinine examination
can be used to describe cigarette smoke exposure among passive smokers. The
presence of positive urinary cotinine results in some respondents indicates that
environmental cigarette smoke exposure still occurs among non-smoking students.
