Abstract

The case study is motivated by the increase in the incidence of Chronic Low Self-Esteem in the West Java Province Asylum in 2020 as many as 24 people, in 2022 there will be 56 people. HDRK is a negative assessment of oneself who thinks that he is insignificant, worthless, helpless, low self-esteem, felt for more than three months. The cause is repeated negative judgments that impact patients feeling isolated, do not want to interact and are always alone so they tend to hallucinate, and commit acts of violence. The purpose of the case study is to describe nursing care for Mr.M with HDRK in the West Java Provincial Hospital. Descriptive case study method with a nursing process approach to one patient. The results of the case study show that in the study the patient said he was embarrassed because he was often ridiculed. The emerging diagnoses are HDRK, Social isolation, Auditory Hallucinations, and Risk for Violent Behavior. Planning for building trusting relationships, identifying positive aspects, assessing abilities possessed, compiling activity plans, carrying out selected activities, TAKS, family care. The implementation of nursing for 6 days showed that the evaluation results of the patient's self-confidence increased. The conclusion in the study is that there is a gap between theory and practice, while diagnosis, planning, implementation and evaluation are in accordance with theory. It is recommended for nurses to increase the frequency of therapeutic communication.
