Abstract

ABSTRACT The incidence of hallucinations has increased in the Asylum of West Java Province in 2022, there were 2082 people, which has increased from 2020 with 1347 people. The purpose of this case study is to describe nursing care for patients with visual hallucinations at the Mental Hospital in West Java Province. The case study design used is a descriptive method with a nursing process approach. It was carried out on one patient with the results of the study showing that the patient said he saw black shadows and heard sounds like broken glass and a broken radio, there was sensory distortion, and the patient sometimes looked at one spot. The interventions include building trusting relationships, helping patients recognize their hallucinations, controlling their hallucinations, making plans for activities at the hospital and at home, participating in TAK activities, helping patients to find out the benefits of taking medication or the effects of therapy, and helping the patient's family to be able to care for the patient at home. The implementation is adjusted to the intervention with the results of the summative evaluation showing that there is an increase in the client's ability to control the hallucinations experienced and the impact on reducing the symptoms of visual hallucinations experienced by the patient. It is hoped that nurses can provide a more frequent approach to carry out communication with patients so that they can help patients reduce the symptoms of hallucinations experienced by patients.
